SIGN UP FOR GRAPE GETAWAYS NOVEMBER 5TH  GETAWAY

I understand the bus leaves from Third Avenue between 44th and 45th Street at 9:15AM, rain or shine. (Exact parking location will depend on availability.) The bus begins boarding at 9:00. It will return approximately 7:00 PM.

The cost for the trip appears below. I realize that by signing up for the trip I agree to accept the terms of the waiver below. If I have to cancel my plans, a full refund will be given for any cancellations received on or before October 31st. Any cancellations received on November 1st or afterwards will receive credit towards a future trip, so long as cancellation notice is received at least 24 hours prior to departure. However, a $30 cancellation fee will be assigned.

You can also pay by check if you mail in your payment so we receive it by October 31. You should also send us your name (and the name(s) of anybody else joining you), your address, phone number(s) where we can reach you and your email address (if you have one). WAIVER AND RELEASE

In consideration of the opportunity to attend the Grape Getaways Wine Tour on November 5, 2011 (the "Event") I hereby waive in all respects the right to bring any action, suit or proceeding against Grape Getaways, its officers, directors, agents and representatives, in any respect relating to the Event, transportation provided as part of the Event or otherwise, and further release and discharge the Releasees with respect to any claim, liability, damage, expense or demand resulting from or relating to the Event. In the event that I bring any action, suit or proceeding in violation of the foregoing, I will pay, upon demand, all legal fees, costs and expenses incurred by any Releasee in connection with defending any such action, suit or proceeding.
